Failure Mode and Effect Analysis (FMEA)

We have extensive experience in the application of FMEA techniques and have the advantage of being able to use practical experience to study complicated systems. This ensures that:
  • Only credible failure modes are highlighted
  • Criticality is objectively decided
  • Practical solutions are proposed
  • Comprehensive report can be issued to third parties
In addition to performing FMEA’s we are able to carry out Fault Tree Analysis that quantifies the critical equipment identified in the FMEA by building a model of the system and the failure paths. By adding failure data to the fault tree components the overall system reliability and probability of failure can be calculated. The benefits of applying this technique in addition to the FMEA are:
  • Overall system reliability is quantified
  • Critical components are identified
  • Failure paths in the system are identified
  • Comparison of different configurations is possible
  • Benefit of changes can be compared
Our recommendations result in cost effective modifications to design and/or procedural changes to minimise the risk of failure.
